Hello, I've just finished AA campaign on medium and I have 19k points (silver medal).
I didn't receive heavy tanks skins.
Do I need finish campaign on hard to get them?

You need to go over 20k points and get the gold from what I remember but someone correct me if it's not true.

20k for a gold medal and tier 3 rewards.

My place in the ladder is irrelevant as long as I am matched with opponents of the same rank and play all factions (balance at place 1000+ is no different from balance in top 100 if we consider what I said before the brakets) so I don't care about your assertion which is also irrelevant.

Nope, and spare requisition at the end of your game easily gives a lot more to your score, so play on easy and skimp on getting commander upgrades to get that 20K+ score.

Difficulty does nothing for your score. It should, but it doesn't. Here's a good guide about it; it has a detailed scoring section:

Getting gold is pretty easy, especially if you do it on easy difficulty. Even if you don't do well on the missions, you get 125 points for a bronze medal and there's about ~15 missions. So, beat the campaign, keep the three companies alive, and hoard at least 80 requisition points, and you'll get gold overall.

No need for guides. All you need to do is save requisition points to get gold which is required for heavy skins. No need to save all, It should be fine with 150 requisition point left at the end. No need to even keep all companies alive if you retain 150+ requisition points.
