Basically, you just need to copy the Soviet HQ's repair_station_ext and the upgrade that activates it to the other HQs. The name is a bit misleading, since you can use it to add either medics or engineers.

yes i copied the rapair station_ext and the upgrade and put it in the german HQ but when i'm activate it
not one medic spawns , where is the problem ?

The upgrade uses a filter_action that only applies to the Soviet HQ. Copy the activate_extension_action from that and add it to the upgrade_bag actions directly, then delete the filter_action.
You can also change which medic to spawn in the repair_station_ext, so that not every faction will have Soviet medics.
