This player in on the opponent team but he clearly attacked him teammate halfway through the game. Despite our warning, he still continued to do so to his teammates.

I second this. Happened to us yesterday. He completely destroyed our bases.Of course refused to surrender afterwards.

I registered for this site only to show this to the community. Not that we can do much about it, can we?

TL;DR
Since he is a repeat offender he gets a longer timeban if you report him correctly. Send an email to enforcement@relic.com with a text where you briefly describe what happened, and the steamID64 or steam acc link of the offender. You also attach these screenshots (name of offender and the teamkilling must be visible on them ofc) as well as the replay to the email.

The email must follow a certain structure so please make sure you read the second link above carefully
